- Przedmiotem zamówienia jest zakup, dostawa i instalacja tomografu komputerowego oraz rezonansu magnetycznego. 2. Szczegółowy opis stanowi załącznik nr 1 do SWZ 3. Miejsce dostawy – przychodnia przy ul. Żeromskiego 13 w Warszawie 4. Wykonawca musi posiadać dla oferowanego TK i MR dokumenty wymagane przez Rozporządzenie MDR 2017/745: a) Certyfikat Zgodności CE wydany przez jednostkę notyfikowaną, b) Deklaracja Zgodności UE wystawiona przez producenta, c) Dowód zgłoszenia / powiadomienia do Prezesa Urzędu Rejestracji Produktów Leczniczych (URPL) lub potwierdzenie rejestracji w europejskiej bazie EUDAMED.
