Key areas of focus will be using innovation to: ? Meet the needs of underserved groups and those with complexity/ vulnerability/ co-occurring conditions ? Increase access at place, ? Increase targeted outreach ? Integrate multi-disciplinary approaches across our local system ? Realise opportunities to embed prevention ? Develop a skilled workforce The estimated total potential contract value (including the extension period) is ?52,853,256. The contract commencement date is 1st April 2024. The initial contract period is 5 Years with possible 2 + 2 year extensions, therefore potentially 9 years in total.
