The people supported by this service will be people aged 18 plus who are experiencing homelessness. However, people under the age of 25 should be considered for the young persons pathway unless this pathway is more suitable in the first instance. The service will work in a trauma-informed way to help people using the service to identify the root cause of homelessness and identify barriers to move-on, producing an achievable plan of support. The service will provide a pathway to move on and set out a plan to move on within 6-9 months and will support through the Council's allocation process or sourcing alternative suitable accommodation within the private rented sector or other Social Housing Provision, as suitable.
