Interested organisations must have experience of supporting adults with long-term conditions to increase their physical activity levels. The pathway project will support system change and collaborative action, to prevent long-term health conditions and enable supportive self-management for people with existing long-term conditions, as well as reducing the risk of recurrent events. The pathway will be targeted at residents of the Opportunity Bucks wards of Buckinghamshire, it will maximise community assets and ensure that people get the right support to become active and improve their health (both physical and mental) and wellbeing. Physical Activity Specialists that work on the project are required to be, as a minimum, experienced Level 3 Exercise on Referral qualified (minimum 1 year supporting adults with long-term conditions and trained in behaviour change using the COM-B model).
