Brief description: We are looking for committed and hardworking qualified Social Worker with Adult Social Work experience working with people with Older people, People with physical disabilities and Sensory impairments, Learning Disability as well as Mental Health issues. You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation including the Care Act 2014, Safeguarding, Continuing Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS. Primarily the role will require support with the waiting list and review list. You will be expected to work with vulnerable adults and their carers with an asset-based and strength-based approach to support them to achieve their outcomes within the wider community. You will need to be confident in undertaking safeguarding investigations, mental capacity assessments, best interest decisions, carrying out outcome focused assessments, review assessments with support planning, to meet their care and support needs. You must be able to work with a diverse group of individuals and with our partner agencies to deliver the best outcomes for our customers and their carers. You must have a degree in Social Work, Dip SW, CQSW or equivalent. Please can the agencies send over a CV, 2x up to date references and a copy of Terms and Conditions.
