The pilot is to trial a new way of delivering home care type support in one area of the city from April 2024. This will involve bringing together Home Care, health care, and local community support services to provide a more consistent and joined up support package that's centred around the needs, strengths, and assets of our residents. A draft of the service specification will be circulated shortly.
