We are anticipating the model to be a phased approach, this will however be determined by the demand during December 2023/January 2024. The phased approach will continue during January to a maximum of six beds. The support delivered will need to be person-centred and outcome based and the monitoring of the contract will be through a fortnightly meeting. People will be discharged from Great Western Hospital seven days a week with a requirement to use the trusted assessor to aid swift discharge within 12 hours.
