The successful provider will carry out energy efficiency home interventions and advice to identified households where a resident has severe asthma within the Greenwood Primary Care Network area (primarily Huddersfield). The service should include a range of interventions, including helping identified households to save energy and money, maximise income where possible and improve the safety, warmth and comfort of their home environment and improve their health.
