These contracts cover commissioning of specialist and non-specialist Nursing Home and Domiciliary Services for NHS Continuing Healthcare (CHC) Service Users by ICBs. The Services are commissioned by the NHS NWL ICB for the care of adults and children eligible for fully funded NHS CHC, including Service Users in receipt of NHS CHC through a fast track pathway and NHS Funded Discharge to Assess Clients. The Services are to be provided in Nursing Homes and at individual client's homes to CHC Adults and Children with Nursing/Residential and Home Care needs in local areas as required. The contract value is on a cost per case basis and is dependant on the patient profile of each contract.
