At which point they should exit on to an at-risk register to be recalled annually by their GP. The Authority are seeking to procure a Lead Provider to deliver all elements of the NHS Health Check activity as one contract. Proposed Contract Start Date: 1st April 2024. Contract length: 3 years with an option to extend for up to a further 2 years. The tender will be run as a single stage Open Procedure. The value of the procurement is above threshold, the subject matter sits within Schedule 3 so falls within scope of the Light Touch Regime of the Public Contract Regulations 2015. The contract notice will be advertised on Find a Tender and Contracts Finder as well as being advertised on SCC's In-Tend Platform Suffolk Sourcing. The tender process will be conducted electronically through Suffolk Sourcing. Tender returns will be evaluated, and contracts awarded based on a price/quality split.
