This service will work with the individual to identify any personal or environmental issues impacting on the person's ability to live as independently as possible in their own home. It will then work to support the person using a strengths-based approach to help them to help themselves. The contract length currently under consideration will be for 3 years, with the option to extend for 2 further period(s) of twelve months. DCC will be considering either a single countywide lot or splitting the county into two geographical areas. DCC will be open to bids from consortia with a lead provider or sole providers who can demonstrate how they would deliver the contract on a local District and Borough based footprint across Derbyshire.
