This tender is seeking a Sub-Contractor to provide early supported discharge services which provides brdging packages of care visits, to enable patients who are medically safe for transfer and no longer needs to be in a secondary care setting of our hospitals for their ongoing care. The contract duration will run from May 2023 until 31st March 2024, and there after subject to funding availability.
