This application is for applications for LOT 2 - Transforming Care Project and is for providers of support to people with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition. Transforming care is all about improving health and care services so that more people can live in the community, with the right support, and close to home. Successful Providers for Lot 2 will provide supported living care and support packages for individuals leaving hospital or at risk of admission to secure in-patient facilities arrangements in the future. If you are already on Lot 2 of this Framework Agreement, you do not need to reapply.
