The service must be 'universal in reach and personalised in response' and work with families with children aged 0 to 5yrs to identify health needs as early as possible, improve access to services and improve health and wellbeing by promoting health, preventing ill health and reducing inequalities in the populations of the London Borough of Hackney and the City of London.
