Market Engagement has been undertaken in advance. NHS North West London Integrated Care Board ("NHS NW London ICB" or "ICB") requires a service to provide care and support to the residents in Hillingdon offering Wellbeing service, Falls Service, High Intensity Users (HIU),Prevention Service and Take Home and Settle Service. Simplifying the access to the extensive range of preventative services from third sector organisations through a single point of access and referral. A wellbeing service supports a range of service users, who without the support, would be accessing and using the capacity with other areas of the health service. This frees up time and resources to enable the other service areas to focus on meeting the needs of the cohorts whilst ensuring timely access to health care services.
