Domiciliary Care refers to care for service users living in their own homes, care is provided at various times of the day to meet individual circumstances and need. The service will be accessible to service users aged over 18 who are registered with a GP within Berkshire West. It is envisaged that the service will be delivered predominately within West Berkshire and the surrounding area. The service aims to deliver a safe, high-quality and person-centered care that is pro-active in continuously improving the services. The service will be accessible to service users aged 18 and over who are registered with a GP within Berkshire West. The service will cover Berkshire West localities including West Berkshire, Reading and Wokingham. The service aims to deliver safe, high-quality and personalised care that is pro-active in continuously improving the service and facilitates hospital discharge as well as End of Life care.
