The service delivers rapid responses to patients in the Harlow Primary Care Network (PCN) and the Loughton, Buckhurst Hill and Chigwell PCN. The service delivers responsive interventions in the patients home with the overarching objective of reducing the incidence of patients presenting at Accident & Emergency departments.
