The New service must provide • Phone-based primary care assessment and support • Home visits from a primary care physician when needed • Primary Care Centres currently based in Hillingdon and Harrow, though the locations may be subject to review • Have integrated systems with providers of 111 services This PIN seeks the views and opinions of those organisations that may be interested in delivering the Service.
