The Contractor will deliver Community Living Support Networks based on the model outlined in the service specification, addressing any barriers to independent living and build community cohesion. The model is based on a vision of included communities, where individuals with additional needs are empowered to thrive. The Contractor will build and strengthen networks of family, social and locality-based support that will prevent housing and health crisis and will respond proactively to long-term needs to prevent costly escalation and dependence.
