[More contracts for small businesses and VCSEs](https://tendernation.io/opportunities/for-small-businesses) ## About this tender The existing service has identified significant opportunities within the East Riding's adult population to prevent, delay or reduce the pressure on ASC through early intervention and timely personalised support to address and improve resident's Mental, Physical and Social health, reduce inequalities and inequities resulting from Access and infra-structure issues in the area. The Council is seeking a collaborative, innovative and altruistic partner organisation, experienced in working with people, communities and partners to deliver time limited, personalised support to residents in a variety of ways and locations. This service is to be based within the East Riding of Yorkshire and will only be accessible to adult residents of the East Riding of Yorkshire. The new Service is smaller, in terms of the combined staffing requirements of link and connectors, than the previous social prescribing contract. This change in requirement is due to the changes in the ways the Authority's services are being commissioned and the inclusion of link workers in PCNs and increased capacity delivered by the NHS in its 10 year plan. This is a light touch regime procurement.
