[More contracts for small businesses and VCSEs](https://tendernation.io/opportunities/for-small-businesses) ## About this tender NHS Calderdale Clinical Commissioning Group (CCG) is seeking to procure the provision of a community dermatology service for the population of Calderdale, offering a 3 year contract with the option to extend for up to a further 1 year and a break clause applicable after year 2. The procurement will be an Any Qualified Provider (AQP) process. This will operate within the wider context of dermatology service provision across the West Yorkshire and Harrogate Health and Care Partnership. The Calderdale Community Dermatology Service is for people whose mild to moderate skin condition cannot be managed by a generalist/GP, and who need to access intermediate care, provided by a multi-disciplinary team. The purpose of the Service is to provide comprehensive and equitable dermatology treatment and care closer to home, in the community, potentially aligned to Calderdale Primary Care Networks. The Service will provide timely and appropriate access to advice, diagnosis, treatment and follow up that meets patients' physical and psychological needs. It will contribute to the delivery of an efficient local Dermatology health system for people registered with a Calderdale General Practice. The Service will operate through a fully integrated, accessible model of care for people, ensuring the specialist elements of the service are delivered within a high quality and cost effective framework through the effective and sustainable use of resources, and streamlined referral and management processes. The Service will be provided by a consultant-led multidisciplinary team which offers: • Clinical triage assessment and treatment • Advice, diagnosis, care and treatment in line with national standards • Shared decision making, based on the NHS comprehensive model of personalised care • Self-management as a core element of agreed care plans • Cohesive and seamless pathways based on patient need • Support for the delivery of high quality cancer services in line with national guidance • Technology that facilitates the delivery of high quality patient outcomes (where it is clinically appropriate to do so), such as teledermatology and virtual clinics Service Providers are encouraged to demonstrate an innovative and flexible approach to the delivery of a community dermatology service that can meet the needs of people and communities in Calderdale now, and in the future, within the wider context of dermatology service provision across the West Yorkshire and Harrogate Health and Care Partnership. Service Providers will need to collaborate and communicate with other Providers and partners delivering the Calderdale, and West Yorkshire and Harrogate, Dermatology health system.
