As an integrated care partnership, the Authority and wider health and care partners across the system want to ensure that all children with ASD/ LD in Hounslow are able to achieve excellent outcomes and reach their full potential, ideally thriving in the communities in which they live and interact. We want CYP in Hounslow with ASD/LD to transition into adulthood independently wherever possible, with sufficient resilience and system support to allow them to achieve their chosen outcomes.
