We anticipate that the number of hours may vary by approximately 10-25% over the lifetime of the contract. This would be by mutual agreement through a quarterly review process. The Nurses will provide comprehensive care to named patients in their place of residence. In the first instance care will be provided between the hours of 8am to 6pm on a 7 day a week basis, exact distribution to be agreed in partnership. Over an agreed period of time the intention will be that the Nurses will work in line with the agreed shift patterns for Neighbourhood Teams. The Trust will align the Nurses to a Neighbourhood Team who will identify the work to be undertaken. In time the Nurses will be aligned to a caseload cluster within the Neighbourhood Team and will work in line with the Neighbourhood Team model of care.
