The three CCGs are working with partners across the system (e.g. B&NES, Swindon and Wiltshire councils, Healthwatch, Virgin Care and the South Western Ambulance Service) to join up health and care services and are moving towards becoming an integrated care system in line with the NHS Five Year Plan.
