The service outcomes are ; • Improved functioning e.g. posture, balance, mobility, orientation and safety awareness • Prevention/reduction of unnecessary hospital admissions and other acute care as a result of a fall • Increased proportion of older people supported in their own homes • Enhanced choice and control over their lives Additional information: Direct award of a contract for a period of 1 year + 1 year to enable the CCG to allow the ICP and system partners the time and opportunity to conduct a full review of future service requirements and operating models.
