In accordance with the next steps of the Forward View, Integrated Urgent Care Service Specification and the Long Term Plan, the Out of Hours Home Visiting Service will treat and care for people with an urgent care need in their own home, protecting them from avoidable harm, reducing the number of ambulance call outs and improve patient satisfaction by supporting them at home to manage complex healthcare needs, including end of life care, thereby reducing the need for hospital admission where it's clinically appropriate to do so. This service will provide urgent home visits when GP surgeries are typically closed, from 6.30pm to 8am weekdays, all day at weekends and Bank Holidays. The anticipated contract duration is 3 years with a Commissioner option to extend by up to a further 12 months. Service commencement will be 1/4/2020. The approximate value is £16m which represents 4 years i.e. should the option to extend be exercised. This is a market engagement process designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a Tender exercise. It is intended as an awareness and communication exercise offering an opportunity for potential providers to understand commissioning intensions.
