The provider will be required to deliver the service through a highly integrated, safe and cohesive service model, reducing unnecessary hospital attendance or admissions and enabling optimal patient outcomes. The specification will outline the Commissioners requirement for the provision of services. The contract term is anticipated to commence on 1st April 2020 for a period of 5 years with the option to extend for a further 2 years.
