Specifically: o Vulnerable members of our communities are supported to be independent and safe o We support and coordinate other organisations to deliver services The Hospital Social Prescribing Service will support the Council to achieve these aims. This service will offer individuals support through a holistic approach to their health and wellbeing. The service will support customers and carers to identify needs and work towards personal outcomes which enable independence and self-sufficiency. The service will signpost individuals towards community groups / resources and statutory services for any needs identified.
