It is based on the principles and techniques that are known as social prescribing. It is well evidenced that social factors are a large contributor to health and wellbeing - many people experience declining health and wellbeing due to social issues (e.g. loneliness, bereavement, unemployment, money / housing problems, low to moderate mental health, managing one (or more) long term condition. By ensuring people are connected to and feel part of their local community we can help them stay independent and well for longer and increase their quality of life.
