The care home may also be outside of the city if it is a package re-start. The successful provider will primarily work within City Hospital and Enhanced Assessment Bed (EAB) providers in Birmingham to facilitate approximately 1200 discharges\* from hospitals and EAB providers to care homes per annum. This will be either a discharge to a short-term EAB; a package re-start to a long-term bed, or a new placement in a care home. The successful provider will also act as an independent and unbiased assessor who will support the Council, care homes, home care providers and Adult Social Care to provide a high-quality service to patients. This service provision will ensure timely assessments are completed for citizens who require residential and/or nursing provision following their hospital or EAB admission thus ensuring a safe and timely discharge from hospital and EAB providers.
