The plans may potentially include the commissioning of domiciliary care as part of a larger and multi-disciplinary service that supports people from the point of discharge. WSCC wishes to seek the views of domiciliary care providers for how these services may be best commissioned. The attached Engagement Notice provides further detail and providers are asked to complete and submit the attached Pro Forma to assist in the planning of this service.
