The service bridges the gap between hospital and home and prevents readmission to hospital. It supports the core priorities of the Local Authority (LA) and CCG in managing safe and timely discharge from hospital , reduction in delayed transfers of care as well as patient flow to assist the trusts Emergency Department (ED) performance.
