The focus of this requirement is to support the specialist community services Solent NHS Trust provides to adults with frailty or who are pre-frail. Solent NHS Trust provides a broad range of services to this patient group which includes, but is not limited to:- - Proactive prevention services working closely with Portsmouth City Council and the non-statutory sector. - Inpatient, outpatient and home-based specialist rehabilitation and re-ablement services. - Crisis support - Discharge to assess support services - Condition/Need-specific support services including specialist community heart failure, respiratory, palliative care and neurological management. - End of life care 2.2.
