This service will provide support to patients and carers who are awaiting or have received a dementia diagnosis through the provision of a named care co-ordinator following the dementia well pathway. The service will need to be available/accessible to all residents of the 2 CCG's with innovative and individual support provided to both the patient and the carer. The service will help to prevent unnecessary admissions to hospital and assist with timely discharge when a patient has been admitted and support patients on the dementia well pathway. If and when this requirement is offered to tender, this will be done via electronic means using the internet. Midlands and Lancashire CSU is conducting this procurement exercise as a central purchasing body for another NHS body with whom the successful bidder(s) will enter into contracts for the supply of the services. Any other public sector body wishing to access the contract may do so only with permission from the contracting NHS body. The contract conditions will be set out in the Invitation to Tender.
