The overall aim of the service is to provide a consultant-led, community-based anticoagulation service which will improve patient access to initiation of anticoagulation therapy and provide monitoring of warfarin across various clinic sites, and in patients' homes for housebound patients. The service will be delivered to all appropriate patients registered with GP Practices in South Sefton and Southport & Formby and will ensure equitable access and quality of service to the entire population group. The service will be responsible for making specialist decisions regarding initiation of warfarin for patients referred into the service and provide ongoing monitoring and review of these patients. This will include regular reviews of TTR with recommended actions communicated to the patient's GP. In addition, the service will provide clinical expertise and input if changes to anticoagulant therapy is required (the service provider will manage any changes to therapy), implement bridging plans, and manage patients prescribed long term low molecular weight heparin (LMWH) therapy. This contract will be for an initial 3-year period with the option to extend for a further 1 x 2-year period. This procurement is for Social and other specific services which are Light Touch Regime services for the purpose of the Public Contracts Regulations 2015 as specified in Schedule 3 of the Regulations ('Regulations') [legislation.gov.uk](http://www.legislation.gov.uk/uksi/2015/102/schedule/3/made) Accordingly, the Contracting Authority is only bound by those parts of the Regulations detailed in Chapter 3 Particular Procurement Regimes Section 7 [legislation.gov.uk](http://www.legislation.gov.uk/uksi/2015/102/part/2/chapter/3/made) The Contracting Authority is not voluntarily following any other part of the Regulations. The procedure which the Contracting Authority is following is set out in the procurement documents. As the CCG is a relevant body for the purpose of the National Health Service (Procurement, Patient Choice and Competition) (No 2) Regulations 2013 these Regulations also apply to this procurement.
