Tailored to the unique needs of specific populations living in the Greater Huddersfield area who are registered with the practice. The provider will provide support to patients to enable a large propoirtionto transfer to a mainstream provider of primary medical services within a 12 month period. The population covered will include: ### Homeless Asylum seekers ### Refugees ### Sex workers The average number of patients registered with the practice should be 1500. The registeredlist should not exceed 1750 or drop below 1250 patients, unless with prior agreement.
