The aim of all the services is to deliver Pathway 3 bed based step down services to provide assessment and reablement services. The Therapy service will be aligned to and work in partnership with the care homes providing care support as part of the overall assessment and reablement programme. The service is to be provided in a small number of care home locations across the 3 CCG areas, and will accept referrals for people who no longer need to be in a hospital environment but are not yet able to return straight home, even with support. The Market Engagement Event will take place in the morning on 9th March 2018 in Leicester city at St. John's House, 6th Floor, Conference Room. Further details will be sent closer to the date.
