It is envisaged that the service will work within the new models of care supporting people's mental health and working in partnership with other Local Care services providing a Multi-Disciplinary Team (MDT) approach for their mental health along with other needs, including physical health, employment, housing and social care and would have a greater emphasis on prevention, self-management, choice, peer support, and partnership with other sectors. The service will provide assessments, short term interventions and care navigation from GP practices and Local Care Hubs. Additional information: The CCGs are seeking expressions of Interest at this stage.
