To meet the assessed needs of the patient as identified in a care plan, in their home, during what is perceived to be the final six weeks of the patient's life (i.e. those that are rapidly deteriorating and therefore eligible for CHC fast track funding). All staff will be trained via an evidence based End of Life Care programme. The service will provide one primary function: - To provide End of life, CHC funded fast track health care in peoples own homes according to their assessed need. The District Nurse that refers to the service will have carried out the appropriate nursing assessment and risk assessments.
