It must be able to show by individual, a single shared care plan, providing a consolidated view of care packages from disparate care settings and source systems; pulling information from core clinical and care IT systems &/or databases across primary care, secondary care, urgent care, mental health, community services, care homes and hospices. The system must be able to join, extract data and provide read only access. Potential providers will need to demonstrate they are able to build the above mentioned, to implement the system, to mobilise on a phased approach and to offer a comprehensive marketing/education programme to those using the system. NB.
