The primary purpose of this commissioned service is to procure outcomes based support which delivers independence, physical and mental wellbeing and employment as detailed in the Care and Support Plan for individuals who have been assessed by Essex County Council as being eligible under the provisions of Care and Support Guidance 2014. The individuals in receipt of this service will be young people with Special Educational Needs and Disabilities, whose support plans will focus on the process of transition to adulthood.
