The Contracts will be for a period of 5 years with an option to extend for up to 5 years (60 months). The ITT will be available from 19th November 2015 and a deadline for responses is 5pm, 21st December 2015. These services must be provided by GPs and Nurses and the provider will be expected to deliver: - High quality Primary Medical Services responsive to the needs of the local population. - Promotion of healthy living with the aim of tackling the underlying causes of ill health. - Essential services. - All additional services. - All Enhanced Services commissioned by NHS England and SDCCG. - Increased access.
