The hub will enable an easily accessible and flexible route for Health Care Professionals to refer patient with Type 1 and Type 2 Diabetes to Structured Education. It will also have the facility to allow patients to self-refer. The aim is to reduce some of the barriers people face in accessing Structured Education, maximizing use of available resources and patient choice.
