This is a process designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a Tender exercise. The aim of the service is to provide a telephone-based behavioral coaching service to support patients with long term conditions improve their ability to self-manage and help to reduce avoidable accident and emergency attendances and hospital admissions. This will be achieved by offering advanced coaching and motivational interviewing techniques over the telephone. Patients can become more confident at managing those issues that have become a block to attaining and sustaining lifestyle changes that in turn improve health outcomes.
