The service will also provide integrated high quality, patient-centered health care services based on the principles of clinical governance. The Provider will ensure that they work in partnership with the Council and the CCG to reduce unnecessary admissions to hospitals and care homes, support hospital discharge and provide integrated and preventative community services. The Provider will require a local base within the boundary of Leicestershire to ensure service delivery.
