To be delivered in two models: Model One - A community and outreach service delivered at premises designated by the Provider Model Two - Will be delivered in the form of a home visit, and will be primarily for carers. The service will be contracted as a fixed term, sole provider agreement, procured using the single stage open procedure. The contract initial term will be for two core years with an option to extend for up to two further years at the sole discretion of the East Riding of Yorkshire Council. The estimated value includes all extension options.
