The purpose of this evaluation is to robustly assess and build a strong evidence base on the impact of social action models on experience at end of life. The Authority will look to quantify causal impact on the quality of experience by measuring: • Patients’ levels of loneliness, satisfaction with social support and wellbeing; and • Family and/or caregivers levels of stress and anxiety.
