The Services being commissioned under this accreditation process will be procured via the Any Qualified Service Provider (AQP) model. The procurement will be broken down into two lots: - Lot 1 Basic Domiciliary Care for Continuing Healthcare Patients to assist with their daily living needs, and; - Lot 2 End of Life Care for those patients who have been fast tracked into the service or transferred from continuing care.
