To improve the patient experience and reduce the number of onward referrals to secondary care To operated a well-planned and clearly articulated care pathway, delivering safe and evidence-based care, supported by a robust clinical governance framework To ensure that referrals are appropriately assessed and that patients are seen by the most appropriate clinician with the relevant skills, right equipment, in a suitable location To comply with all relevant waiting times standards and ensure that all stages of the patient’s care are delivered in a time-frame suitable to the patient’s clinical, emotional and social needs To offer services in 2 or more locations, easily accessible to the population of South Gloucestershire This is a 36 month contract with the possibility to extend for up to 24 further months subject to commissioners discretion.
