The procurement will be in the following lots: - Lot 1 Kirkby Community Primary Care Centre – Kirkby in Ashfield, Nottinghamshire and - Lot 2 Balderton Primary Care Centre – Newark, Nottinghamshire . NHS England and Commissioners are looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets. The model will need to include provision of the following: • Essential Services • All Additional Services • Directed Enhanced Services • Development of innovative service provision with Clinical Commissioning Groups around strategic ambitions which include integration, collaboration and use of technologies The Contracts will be for a period of 5 years with an option to extend for up 5 years (60 months).
